ORDER FOR THE 2004-2005 SCHOOL YEAR NOW!
To continue receiving Nebraska Trailblazer, the history paper produced by the Nebraska State Historical Society, complete the order form and return by March 1, 2004. If you did not receive an order form in the mail or it has been lost, contact John Lindahl, 402-471-4764, museum02@nebraskahistory.org. On-going support for printing and shipping the Nebraska Trailblazer is provided by the Thomas Creigh, Jr. and Dorothy Weyer Creigh Endowment established with the Nebraska State Historical Society Foundation through gifts from Thomas Creigh, Jr., in memory of his wife.
To order free Nebraska Trailblazers, select six issues from the attached list for use in all fourth grade classrooms. Order one Nebraska Trailblazer for each fourth grade student, plus copies for the classroom teacher(s) and media center.
Only one order form is sent to each school. Please return this form by March 1, 2004, since we will print only the quantity ordered for each issue. You will need to use your best judgment about how many fourth graders will be enrolled in the fall of 2004. Orders must be received by March 1. If orders are late we cannot guarantee you will receive the issues you have selected, and substitutions will be made.
In addition to the six free issues you may purchase additional copies by completing step four on the order form. There is a $6.00 shipping and handling fee for all orders, FREE OR PAID. Payment for the handling charge, plus the amount for additional Nebraska Trailblazers ordered, must be included with your order.
Complete the order form and send it with your payment to Nebraska State Historical Society, P.O. Box 82554, Lincoln, NE 68501-2554. Orders received without payment will not be filled until payment is received, as Nebraska state law requires payment in advance. Local and state taxes must be paid if the check for payment is not drawn on a tax-exempt institution. The school name, address, and order you enter on the order form is the information we need to complete your order. Confirmation that we have received your order will be sent.Important points when ordering:
- There is a $6.00 shipping and handling charge on all orders, free or paid.
- Enclose check with order made payable to Nebraska State Historical Society. No orders will be shipped without payment.
- Your school's order must be received by March 1, 2004
- Select only six issues for all fourth grade classrooms in your school,
